Returning Materials to Epic

IMPORTANT: This page contains two separate forms. In some instances, it will be necessary to complete both forms. Please complete the necessary form(s) pertaining to your family by clicking the links below.

There are several ways to return our devices! You can complete one of the forms below or you can bring them to our Tulsa office, Oklahoma City office, or one of our Learning Centers.

You can also take them to any UPS Store in Oklahoma with the instructions and return code found in the link below. If you only have a UPS drop-off location in your local area, we can send you return materials, just complete the form below.

Please put laptop in bag and power cord in open slot. Make sure items are secure with packaging material to avoid damage in shipment. Take package to your nearest UPS shipping center.

Please put ipad in protective case and charger in open slot. Make sure items are secure with packaging material to avoid damage in shipment. Take package to your nearest UPS shipping center.

Please put internet device(s) in protective case and cable in packaging material to avoid damage in shipment. Take package to your nearest UPS shipping center.

Always retain a paper or electronic copy of your receipt. If paper, save it electronically by taking a picture.

Be sure to include accessory items if returning a laptop or iPad.

If "Yes" then a label will be emailed to the email address you provided above. You will need access to a printer to print the label. If "No" then a label will be mailed to the physical address you provided above.
Smaller Laptops: The number is above the DELETE key.

Regular Laptops: The number is above the ESC key.

iPads: The number is found on the back of the device.

Customer Instructions:

  1. Locate the nearest participating location by either of the following two options:
    • Go to theupsstore.com/locations, enter address information and choose the most convenient location.
    • Or call 800-789-4623 and request the nearest UPS store location.
  2. Please take this document to the UPS Store location and tell the center associate you are part of the Corporate Retail Solutions Student Returns program for EPIC Charter Schools.
  3. This program has been designed to provide Pack and Ship services.

Instructions for the UPS Store Location:

  1. This customer has an electronic device(s) to pack and ship through CAMS.
    You will need RETURN CODE: RETURN2EPIC to process workflow.
