RETURNING MATERIALS TO EPIC

IMPORTANT: This page contains two separate forms. In some instances, it will be necessary to complete both forms. Please complete the necessary form(s) pertaining to your family by clicking the links below. Also, this is for Asset Returns only, if a replacement is needed, please submit a ticket to the Asset Department.

There are several ways to return our devices! You can complete one of the forms below or you can bring them to our Tulsa office, Oklahoma City office, or one of our Learning Centers.

 

You can also take them to any UPS Store in Oklahoma with the instructions and return code found in the link below. If you only have a UPS drop-off location in your local area, we can send you return materials, just complete the form below.

 

Please note that the UPS Store option is only for technology returns (Laptops, iPads, MiFi’s, and Calculators)

Asset Return - MiFi/Laptop/iPad/Calculator returning assets. If you only have a UPS drop-off location in your local area, we can send you return materials, just complete the form below.

Asset Return – Learning Fund Items

Returning assets, other than technology. If you only have a UPS drop-off location in your local area, we can email you a prepaid return label to print out, just complete the form below. You will need to provide the box for these items.

Instructions for Returning at UPS Store

Customer Instructions:

  1. Locate the nearest participating location by either of the following two options:

    • Go to theupsstore.com/locations, enter address information and choose the most convenient location.

    • Or call 800-789-4623 and request the nearest UPS store location.

  2. Please take this document to the UPS Store location and tell the center associate you are part of the Corporate Retail Solutions Student Returns program for EPIC Charter Schools.

  3. This program has been designed to provide Pack and Ship services.

Instructions for the UPS Store Location:

  1. This customer has an electronic device(s) to pack and ship through CAMS.
    You will need RETURN CODE: RETURN2EPIC to process workflow.
